Social Prescriber Based at Roundstone Surgery | 16 Hours per Week | Flexible Working

Are you passionate about helping people improve their health, wellbeing, and quality of life?

Trowbridge PCN is looking for a compassionate and motivated Social Prescriber to join our friendly team. Based at Roundstone Surgery, you will support patients to access community services, activities, and practical support that can make a real difference to their lives.

You will work closely with GPs, practice teams, and local organisations to help patients overcome challenges such as loneliness, financial concerns, housing issues, and long-term health conditions.

This is an excellent opportunity to join an established and supportive Social Prescribing team, with guidance and peer support from other Social Prescribers employed across the PCN.

We are looking for someone who:

Has excellent communication and people skills.
Is empathetic, organised, and able to work independently.
Has experience in health, social care, wellbeing, community, or voluntary sector settings.
Is passionate about supporting people to achieve positive outcomes.
